Student Forms

  • Student Forms for the 2020-21 school year will be sent to you in July, 2020. 
  • If you are a newly enrolled student for the 2019-20 school year, you will receive a link to your student’s required school forms. Completion of these forms is required prior to the first day of school. 


Statement of Health Forms 

These forms need to be completed and signed by your child(ren)’s physician or health care provider and returned to your campus. In addition, a current immunization record is required.

Onsite Medication Forms


There are strict guidelines for properly filling out each medical form as required by the state of Colorado. Be sure to carefully follow the Instructions for Parents and Physicians attached to each form. MSE cannot accept incomplete or incorrect medical forms. The safety of your children is our utmost concern. 


Classroom Volunteer or Field Trip Driver

If you are interested in volunteering with students or driving on a field trip, you will need to complete the Background Authorization/Driving Record Form if you haven’t already. This form only needs to be completed every other year, so please check with your Office Specialist to see if your authorization is current. If you are not current, they will send you the link to complete the form. If you are planning to drive on a field trip, we will also need a copy of your current driver's license and current proof of insurance.


School Lunch Program Sign Up

  • Sign-ups closed for the Spring 2020 Semester. Look for the Fall 2020 Sign-up at the beginning of the 2020-21 school year. 



Bus Transportation and Before and After Care Programs

  • The MSE bus is transporting students between campuses both before and after regular school hours. 
  • MSE offers Before Care at both Marshdale and Troutdale Campuses from 7:45am until 8:30am and After Care from 3:00pm until 4:00pm for the short program and 3:00pm until 5:00pm for the long program. Your child must be five years of age or older to participate in our Before and After Care Program.
