Medical Exemption Form

Nonmedical Exemption Form
More InfoA nonmedical exemption (NME) is an exemption to one or more school-required vaccines based upon a religious belief whose teachings are opposed to immunizations or a personal belief that is opposed to immunizations. This process may be completed online by parents/guardians, emancipated students, or students 18 years or older.
General Medication Administration Form
FormThis form is to be filled out for students who require prescription or over-the-counter medication (OTHER THAN inhaler, epinephrine or antihistamine) to be administered during school hours or on a
field trip (including overnight trips).
Allergy & Anaphylaxis Administration Plan
FormColorado state legislation requires that strict and specific documentation and practices must be in place before any staff can administer any medication to your child, for both prescribed and over-the-counter
medications.
Asthma Plan & Administration Form
FormThis form is to be filled out for students who have asthma, which may require the adminitration of an inhaler during school hours or on a field trip (including overnight trips).

TODDLER, PRIMARY, & KINDER
FormPlease have your child's doctor fill out this form prior to the start of each school year. Once form is signed and returned, you can send it to the Office Specialist at your child's building to keep on file.
